Registration for the Elks Lodge #613 Local Area Hoop Shoot Contest (2026)
Please complete this form to pre-register for the 2026 Santa Barbara Elks Lodge #613 Annual Hoop Shoot Competition. This form must be READ and COMPLETED by the parent or guardian of the intended participant.

Competition Groups: Each age and gender group for the competition is limited to 10 participants.

Age Verification: A copy of an age verification document is required to participate.  See details below.

Basketballs: Participants may bring a basketball to warm up but regulation basketballs are provided for the competition. 

Registration: 11AM-12PM.  The competition will start promptly at 12PM.  Save time at registration by registering in advance with this form.

Proof of age is required to participate in the Hoop Shoot.  As a parent or Guardian, I will provide A COPY (no originals please) the following form of verification and will bring it to the competition. *

Hoop Shoot Photo Release

I understand that my child/youth participates in an Elks youth activity program, and that the activity may be of public interest, and that I hereby consent and authorize the Benevolent & Protective Order of Elks (BPOE) and Santa Barbara Elks Lodge #613 (collectively, the “Elks”) to use and reproduce my child/youth’s name and/or image or likeness, and to circulate the same for all purposes related to the conduct and promotion of the youth activities of which my child/youth is a participant.  This includes but is not limited to Elks newsletters, flyers, brochures, posters, website advertising Elks youth activities to the public and/or the Elks Lodges’ social media pages (Facebook, Instagram, etc).


I further consent and authorize the use of my child/youth's name and/or image and likeness to be released to the press for all purposes related to the conduct and promotion of Elks youth activities of which my child/youth is a participant.  This includes, but is not limited to the newspapers, television and/or radio.
