READ CAREFULLY: This form is for ONE single applicant. If you are sponsoring another applicant (even a spouse), you will need to complete another form out with their specific info. Each applicant needs TWO separate sponsor forms.

In order to sponsor someone you must be:

- a member in good standing (up to date on membership dues)
- a member for at least 6 months
- willing and able to help them get acquainted with the club and its policies.
