Camacho Program Withdrawal Form
Use this form to cancel your registration for a program or activity.

  • Requests are processed in the order received, within 72 hours.
  • All cancellations must be submitted at least 7 days before the program start date to be eligible for a credit or refund. Please refer to the program listing for specific policy details.
  • For multiple cancellations, submit a separate form for each participant or program, or include all details in the comment section below.
  • Please add any important details we may need to know about your cancelation in the comment section. Credit is issued by default, if a refund to the original payment method is preferred, please leave a note.
