【SOUND STUDIO DOM】新規イベント予約フォーム

必要事項にお答えいただき、フォームを送信後に予約完了となります。

以下を必ずお読み下さい。

一度あらかじめ当店までお電話頂き、開催日の空き状況をご確認頂くと、スムーズに対応できるかと思います。
あくまでイベントを打つ事が前提となりますが、日時の変更はできる限り柔軟に対応致します。
キャンセル料金は15日前までは無料、2～14日前までは50％、前日当日は100％、キャンセル料金を頂戴致します。
初回利用の方はご来店(14:00~22:00)が必要な場合が御座います。
フォームを送信後にメールにて返信、企画書と同意書を送らせて頂きますので、必ず利用できるメールアドレスをご入力ください。
