Hoops 4 Kidz is open to boys and girls in grades 5th - 8th grade who play on a St Marcus school basketball team

SESSION DAYS & TIMES
Tuesdays/Thursdays 4:00pm
capped at 20 per session. Sessions may be added if needed

COST
$25 per week
This allows us to say thank you to our professional trainer as well as help with equipment upkeep

I agree that on behalf of the enrolled student named on this application form, St. Marcus and/or camp staff or employees will not be held responsible for any injury, accident, or loss of property, however caused.  It is further agreed that all risks involved in participation in said camp are assumed by the student and his/her guardian, who are responsible for the medical fitness of the enrollee and for all medical costs incurred in case of injury while in attendance at St. Marcus programs, camps, leagues, and/or activities.  By signing you are also giving your consent of advance for emergency medical treatment, should it be deemed necessary by the program director(s).
