This form is for regular busing for incoming Kindergarteners through 12th graders for the upcoming school year. Please use this form if you meet any of the following conditions:
1. 11th or 12th grade student that needs busing (otherwise not provided)
2. You live within the walk zone and need busing from bus stop or alternative address like a daycare outside the walk zone (based upon availability; there is a fee)
3. Open enrolled and want busing from bus stop within district (based upon availability)
4. Busing is needed to an alternate bus stop (ex. Family 2 address, childcare, 2 family homes, etc.)
5. No busing is needed

If you do not meet any of these conditions, busing will automatically be set up for you to/from your Family 1 address listed in Skyward and there is no need to fill out this form.
