Moving Mutts and Mousers  Request for Transport


IF YOU ARE A RESCUE AND THIS IS THE FIRST TIME YOU HAVE REQUESTED TRANSPORT ASSISTANCE FROM ME, I MUST SCREEN YOUR RESCUE FOR APPROVAL.  YOU MUST COMPLETE THE MOVING MUTTS AND MOUSERS RESCUE SCREENING FORM.  EMAIL PAULA at prbford@fastmail.fm to ask for a "New Rescue Form".

PLEASE PROVIDE COMPLETE INFORMATION - INCOMPLETE FORMS WILL NOT BE ACCEPTED!  Full transport rules and requirements are detailed below and at end of form.  Before beginning request form you need to have each animal's veterinary records for reference to provide specific dates for vaccines, etc. that are required to complete form.

If you are requesting transport your completed form MUST be submitted to me NO LATER THAN 10 DAYS PRIOR to the weekend of transport. If you have already been in contact with another coordinator, please do not request my help as well, unless you have received confirmation from that coordinator that they can no longer provide assistance. We as Transport Coordinators work together so there is nothing that I can do any better than your Coordinator is already doing.


Tagging rescue/receiving party accepts ownership of the animal(s) at the time the animal(s) is tagged.  The rescue/receiving party agrees to provide funds for veterinary requirements and is fully responsible for the animal(s) tagged during transport.  This includes acceptance of financial responsibility for any/all veterinary care if an animal requires veterinary care during transport for any reason.  Further, receiving party/rescue may NOT refuse an animal(s) once on transport.  If there is an issue/discrepancy with the animal once he/she is received by the receiving party/rescue, the receiving party/rescue must deal directly with the sending party to remedy the situation.  AT NO TIME, NOR UNDER ANY CIRCUMSTANCES, WILL AN ANIMAL BE ABANDONED DURING TRANSPORT!  TRANSPORT COORDINATOR AND TRANSPORT DRIVERS WILL NOT BE RESPONSIBLE FOR ANIMALS(S) WHO ARE REFUSED BY RECEIVING RESCUE, NOR FOR ANIMAL(S) FOR WHOM RECEIVING RESCUE DOES NOT SHOW UP TO CLAIM AT AGREED UPON TRANSPORT DESTINATION MEET TIME AND LOCATION.  RECEIVING PARTY/RESCUE TAKES FULL OWNERSHIP AND ALL RESPONSIBILITY FOR ANIMAL(S) BEING TRANSPORTED FOR RECEIVING PARTY/RESCUE.

Except in emergency situations, all animal(s) going on transport must have been quarantined outside of a shelter/kennel environment for 7-23 days, depending on age, health status, and exposure of the animal.

I reserve the right to refuse the set up of Transport to any rescue or individual.



REQUIREMENTS FOR TRANSPORT - PLEASE READ THOROUGHLY
 
1.  ALL PROVISIONS PER THE USDA ANIMAL WELFARE ACT AND ANIMAL WELFARE REGULATIONS MUST BE ADHERED TO - NO EXCEPTIONS.  (Reference USDA Animal Welfare Act and Animal Welfare Regulations)

2.  All animals MUST BE up to date on shots (canine and feline) as appropriate for the animal’s age.  All animals must be documented by a licensed veterinarian to be in good health, including being free from internal parasites, fleas, ticks, diarrhea and kennel cough. This may require that the animal(s) is boarded locally prior to transport; sending party or receiving party will be financially responsible for necessary boarding.

3.  Except for medical emergency transports, all dogs must have been quarantined OUTSIDE of the shelter environment for 7-23 days.  Adult dogs (12 months old & older) require completion of minimum 7 days quarantine prior to transport; puppies (animals less than 12 months old) require completion of up to 23 days quarantine prior to transport.  Dogs MUST NOT be exposed to unquarantined or unvetted dogs during or after their quarantine period.

4.  No animals will be “added on” to transport without prior authorization from Paula Ford.  Any animal added to a transport must meet all requirements for transport.

5.  ANY EXPENSES INCURRED IN PREPARING THE ANIMAL(S) FOR TRANSPORT ARE THE SOLE RESPONSIBILITY OF THE RECEIVING PARTY/RESCUE UNLESS OTHER ARRANGEMENTS HAVE BEEN MADE.  PAULA FORD, MOVING MUTTS & MOUSERS TRANSPORTS, OR DRIVERS WILL NOT BE RESPONSIBLE FOR ANY UNPAID EXPENSES FOR ANIMALS BEING TRANSPORTED.

6.  All animals must travel with a current and valid original rabies certificate or documented current rabies vaccination  (if old enough for rabies vaccine).

7.  All animals traveling across state lines must travel with a valid Health Certificate issued and signed by a licensed veterinarian.  A Health Certificate is NOT the same thing as a vaccination and/or health records.

8.  TRAVEL DOCUMENTS MUST BE FAXED TO ME NO LATER THAN 6 PM ON THE DAY PRIOR TO THE DAY OF TRANSPORT.  If I do not have your documents, your dogs will not travel.  Travel documents include rabies certificate, health certificate (for all inter-state travel), and record of vet care to include veterinarian examination, vaccinations, heartworm status (if old enough), spay/neuter status, last deworming, and last flea treatment.

9.  Animals will not be transported sooner than 7 days after vaccinations of any type.

10. Animals will not be transported sooner than 7 days after neuter surgery or 10 days after spay surgery.

11. All animal(s) who are less than 6 months of age and/or less than 30 pounds MUST travel in a crate; crate must be provided by receiving party or sending party.

12. If crates are required for animals to travel, crates must meet Federal Animal Welfare Act requirements such that crate/primary enclosure must be large enough to ensure that each animal contained in the crate/primary enclosure has enough space to turn about normally while standing, to stand and sit erect, and to lie in a natural position.    

13. No animal(s) less than 6 months of age or less than 30 pounds may travel tethered.

14. No more than one animal of sufficient age and/or weight may travel tethered in the same vehicle unless they are able to be completely separated.

15. Animals traveling tethered MUST come with a harness, in addition to a collar, for use in tethering.

16. It is preferred that Martingale collars be utilized.  Animals may be nervous and act uncharacteristically while on transport and may slip a flat collar.  Please utilize Martingale collars if at all possible.

17. A quality slip lead (NOT the nylon type used in vet’s offices/grooming facilities) is required to be sent for each dog in addition to the clip-style lead. (Clip leads will be used on the harness.)

18. Small dogs (less than 30 lbs) must come with a well-fitting harness and leash even though they are required to travel crated; ALL cats must come with a well-fitting harness and leash.

19. All animals must come with appropriate identification on collar – NO EXCEPTIONS: Animal’s name/ID number and complete Receiving Party/Rescue information (Receiving party address, contact name, contact phone number, and contact email).  I recommend using the paper or plasticized ID bands like the vet’s office uses, since tags on collars can become caught in the crate.

20. Animals older than six months of age and of the opposite sex, who have not yet been altered, will not be transported together unless they are crated separately.

21. Puppies less than 6 months of age and/or who have not received three series of puppy vaccinations (distemper/parvo combo - “DHPP”) at least three (3) weeks prior to transport date MUST travel in a crate; they will travel “No Paws On the Ground”.  Drivers are instructed on “NPOG” protocol such that the pups’ feet will NOT touch the ground during transport.

22. Puppies less than one year old must have received at least 2 of the 3 vaccines in the puppy series to travel.

23. Puppies from different litters may not travel together on transport unless all puppies have lived together in the same immediate environment for the duration of the entire required pre-transport quarantine period.

24. Puppies less than 8 weeks of age may not travel on transport.

25. Animals 4 months of age or less may not be transported in the same primary enclosure (crate) with adult animals other than their dams.

26. Any female animal in heat (estrus) may not be transported in the same crate/primary enclosure with any male animal.

27. Cats must come with a small litter box in their crate.  Pine-type litter is preferred over gravel.

28. I do not require Sending Shelters to assist with first leg; if you can find someone that would be helpful.

29. If Sending Party is a Rescue/Foster/Owner Surrender I DO require that you drive the first leg of transport or find someone who can; this shows good faith.  If you are unwilling to assist in moving your animal(s) then I can't help you.

30. I also require that the Receiving Rescue/Adopter/Foster drive the Last Leg of Transport; again if you can't do it you need to find someone who can, if you are unwilling to assist then I can't help you.

31. *** A picture of the animal must be emailed to me at prbford@fastmail.fm for use on the transport run sheet when the request for transport form is submitted.

32. *** Contact/driver information for the driver doing the first leg out and the last leg in for the transport must be supplied to me with this request for transport form.
 


If at any time during the Transport Process (i.e., During Transport "Legs", Overnight Stay, Or Temp Foster to continue Transport), the animal becomes ill and requires medical attention it will be the responsibility of the Receiving Party/Rescue to pay any/all costs incurred for treatment.  Paula Ford, Moving Mutts & Mousers Transports, or Volunteer drivers/overnighters/temp fosters will not be responsible for expenses.
 
Adoptions directly off the transport are not permitted under any circumstances.

Anyone wishing to adopt an animal they transport must go through regular channels per the receiving rescue.
 
On occasion a transport route may not fill completely and the transport will be at risk of cancelling.  Rescues are hereby advised that it is expected that the rescue will provide gas money, in the amount of $20.00 per leg for more than one leg driven by a single driver, to drivers in order to prevent the cancellation of the transport.  Additionally, if a driver needs funds in order to fill a critical transport leg and thereby prevent the transport from cancelling, rescues are advised that it is expected that the rescue will provide gas funds to prevent cancellation of their transport.  This is only expected in situations where there are only several legs left unfilled; less than 25% of the total number of transport legs on a given transport.  Rescues always have the option to cancel the transport and/or utilize another Transport Coordinator.
 
While drivers are advised as to the appropriate handling of the dog(s) during transport to ensure their safe arrival with rescue, accidents may happen.  In the event a dog is separated from transport, Paula Ford will make every effort to return him/her safely to the group.  This may include but is not necessarily limited to: notifying local police, animal control, veterinarians, and shelters; recruitment of local volunteers to post signs and assist in physical search (IF Possible); registration with http://www.lostandpound.com/;http://pets.groups.yahoo.com/group/K9AmberAlert/, and other known lost pet sites. Rescue will not hold Paula Ford or volunteer driver(s) liable for damages or other monetary losses as a result of loss or injury to dog while on transport.
 
INCOMPLETE FORMS WILL NOT BE ACCEPTED AND YOUR TRANSPORT WILL NOT BE SCHEDULED.  
 
Name, phone number and contact person for receiving party/rescue and veterinary information must be provided.  If animal(s) going to Approved Adoptive Home I must have the name/website and contact person for the rescue who did the Home Visit, Vet Information,  i.e. name, phone, and contact person.  I will not transport animals to an adoptive home for which references have not been checked and a home visit check has not been performed.  ALL animals going to Adoptive Homes MUST be spayed/neutered unless written documentation, signed by licensed veterinarian, is provided on letterhead stating medical reasons this cannot be done and faxed to me at 252-679-7298  
 
Submission of a transport request form to Moving Mutts and Mousers Transports is NOT a guarantee of transport.
 
Moving Mutts and Mousers is an all volunteer-based group so the transport goes if the legs fill; if it does not fill, it may be rescheduled until filled, until you cancel the transport, or unless I deem that I will not be able to secure drivers for the transport.  As stated this is an ALL Volunteer based group. We are NOT a 501(c)(3), we do not charge a fee for transport, but do accept donations through PayPal - account is movingmuttsandmousers@gmail.com ; donations should be specifically noted as being for a particular transport (all my transports have a name).  Please select “Personal” when you send so there are no fees charged.  All monies donated go to helping sponsor transports with gas funds for drivers if needed.  THIS IS DEDICATED GAS MONEY FOR TRANSPORTS ONLY.  If you donate and funds are not needed for your transport, funds will be used for future transports.
