Welcome to the Ponderosa High School Referral Form! Referrals are submitted electronically to the Assistant Principal's Office and Dean's Office for review. Please fill out the form completely and hit the SUBMIT button at the bottom when completed.
If you are sending a student to the office, please call Kellie Rath at Extension 2211 to notify a student is en route and whether you would like that student to return to your class. If you have a student who is refusing to leave class, please call Kellie Rath so a campus monitor can be sent to your classroom for assistance. If you need assistance from a campus monitor, but require discretion, please send an email to Kellie Rath and explain the situation.
Once your referral is submitted, Dean and AP Secretary will review the narrative and call the student to the office accordingly. Dean will conference with student for the following referrals:

Dean will also meet with students regarding dress code violations, students out of bounds (students in parking lots, student vehicles or other designated out of bounds areas) and students off campus. If you have these concerns, please email Kellie Rath.
