This form is an application to apply for a parking permit. Parking at DHSW is a privilege. There is no guarantee a student will be issued a permit. The 2026-2027 Parking Permit guidelines will be communicated by the administration team prior to the start of the school year. You will not receive an approval or a response to any inquiry prior to August 10, 2026.   

This form is only for DHSW students, for the Classes of 2027 & 2028.  All other entries will be deleted. 

You will also need to print and sign the 2026-2027 Parking Rules- DHSW.

Seniors need to have PRESENTED their Graduation Project to be approved for a parking permit. 

Juniors need to have PROPOSED their Graduation Project by November 2026 to be approved for a parking permit.

 All assigned fees pertaining to high school needs to be paid to be approved for a parking permit.  *
To be approved for a parking permit all obligations must be cleared. The obligation letter was emailed during Summer 2026.  *
A copy of your responses will be emailed to the address you provided.
