Neighborhood Hero Nomination Form
The Love La Mirada community service organization wants to honor those members of our neighborhoods who are already living out our mission to see "neighbors serving La Mirada together" by receiving nominations from La Mirada residents from various areas around the city.

Please respond to the following questions so that our board can review the information about the person whom you've seen serving in your neighborhood.  (NOTE:  All nominees must be residents within the city of La Mirada.)

We will select ONE "Neighborhood Hero" from the list of nominees at our monthly LLM Board meetings and reach out to them with a gift card & acknowledgment of gratitude on behalf of you and your neighbors.
