Release of Liability: In consideration of permitting the above named child to participate in games, practices and other activities of the Youngstown Recreation Program, I. the undersigned as parent or guardian of said minor, do hereby, release and agree to hold harmless the Youngstown Recreation Program and its said agents, employees and coaches from any liability for personal injury or property damage which may occur to said minor as a result of his/her participation or as a result of negligence on the part of said program or its agents, employees and coached related to the program.
