Previously, an authorized individual, together with an approving Administrator’s confirmation from the Company Name you provide below (“Customer”), had granted authorization to Geotab to share VIN Data, GPS Data, Accelerometer Data, Trip Data, and Odometer Data from all devices in its Geotab database (“Telematics Data”) with InShare Services Inc. (“Partner”).

On behalf of Customer, you are confirming your revocation of any previous authorization requests and requesting that Geotab stop sharing Company’s Telematics Data with Partner.

Once this revocation form has been received by Geotab, it will be processed and Geotab will start the process to stop sharing the Telematics Data.
