Minor Child Photo Release Form

I, (Caregiver Name), the parent or legal guardian of (Child's name), grant my permission for my child to be photographed during Positive Movement Entertainment classes/ events. Furthermore, I understand that no royalty, fee, or other compensation shall become payable to me for such use. Legal use includes, but is not limited to, promotions, publicity, copyright purposes, illustration, advertising and web content.

Code of Conduct

All members must sign and agree to follow the same rules. Your signature means you understand that breaking the rules will have consequences. 

1. SAFETY FIRST! Always be respectful to others, material, and YOURSELF!

2. Must talk with manners (if you are under 21 adults are Mr. and Ms.)

3. (Yeah) is a curse word you must say YES

4. No FIGHTING, FRATERNIZING, cursing, horse play, etc.

5. You may not join any other team without giving notice to directors.

6. No weapons of any kinds are allowed at practice or events.

7. No spectators- only members are allowed at practice. People who are interested in joining the team at registration times.

8. All participants are expected to attend meetings. Parents must make it to at least one meeting.

9. All members must be a part of fundraisers and big events.

10. No unexcused absences (drill team is like a job, your presence is very important).

11. Parents' support and participation is a must for your child and team to succeed. Caregiver support is expected at all practices and PME events.

12. If you do not follow rules you will not be able to travel with us to events until further notice.

13. YOU MUST KNOW AND LIVE BY THE PLEDGE! NO EXCEPTIONS!!
