Conrad Weiser East Elementary Students GRADES: 2-4
Conrad Weiser Tennis is EXCITED for the return of AFTER SCHOOL Tennis programs at CW East and CW West Elemantary Schools this spring. As part of NATIONAL TENNIS MONTH in AMERICA- CW Tennis is bringing
AFTER SCHOOL Tennis to each of their Elementary Schools.
DATES: MONDAYS- May 9, May 16, May 23
TIMES: 3:30pm-4:30pm (Students will dismiss from their class and go directly to the CW East Gym. At the end of the program on each day, students will be dismissed by grade levels at the front of the school entrance under the awning. Parents MUST be present at 4:25pm to sign out their child each day. IF a student is going home with another student and their parent- parents must check this off on the registration form.
LOCATION: CW East Gymnasium (players will leave their class at 3:30pm and go straight to the Gym.

ABOUT THE PROGRAM:
CW Tennis is look for NEW student-athletes GRADES 2-4 that are interested in trying tennis for the first time.
The program, following the CW Tennis progressive pathway program of Tennis in PE, After School Tennis Programs and year round programs for Elementary (grades K-4), Middle School (grades 5-8), High School (grades 9-12).
Players that ENJOY the CW Tennis After School Program in May will have the opportunity to learn MORE about and join the the CWTA First Serve 10U Program (Grades K-4) AND for 4th Graders (going to 5th grade in Fall 2022) the Jr. Scouts program both of which are year round programs.

I understand that I will not hold Conrad Weiser Tennis Association, Conrad Weiser Athletic Boosters (CWAB), Conrad Weiser School District (CWSD) and all members of its board or others associated with this event liable or responsible for any claims, demands, losses, suites, liabilities and cost resulting from bodily injury or theft of my personal property during the event.
Participation involves a risk of injury. By signing this form, you represent that you have considered the risks of participation in the activity and have obtained any medical clearance necessary to participate. In the event of injury, program supervisors shall administer basic first aid and shall summon emergency services via 9-1-1 if needed. Supervisors are not trained to provide detailed medical care. All costs of emergency care are the responsibility of the participant/participant’s legal guardian(s).
I hereby grant CWAB and/or their legal representative permission to use photographic portraits of the event, that may include the participant, in publications, forms, advertisements, newsletters, and websites. I waive any right I may have to approve the finished product. I hereby release CWAB and/or their representative, or those for whom they are acting, from any liability for any violation of any personal or proprietary right I may have in connection with the use of the above stated images.
Participants agree to follow all rules of conduct established in conjunction with the activity. Failure to follow the rules will result in your removal from the event and you agree that any program fees paid, if any, will be forfeited.
