 Liability Release
By signing my name on this form, I understand that I will not receive any compensation or payment for my services, or any other thing of value in lieu of compensation, and that I am a volunteer and not considered to be an employee of TCWP.  I further recognize that there may be certain inherent risks associated with the activity / event named above and I assume full responsibility for personal injury to myself and further release and discharge TCWP for injury, loss, or damage arising out of my volunteer activities for TCWP, whether caused by the fault of myself or other third parties.  I indemnify and release TCWP from all claims, causes of action, damages, judgments, costs, or expenses, including attorney fees and other litigation costs, which may in any way arise from my volunteer activities for TCWP.
