In order for us to have the most accurate member information, please complete the following form. This form is required of all members. In order to be an active member, this form must be completed and submitted prior to payment. Thank you for your participation!

Payment: Registration Fee is $30 for this school year and will come with a complimentary club shirt. If registration is not done prior to October 3, this fee will alter to $28 NOT including a club shirt or $35 with a club shirt. Payment must be made before your membership is active and before participation in club-sponsored events is allowed. More information about payment system will come soon! 

Please Note: Simply filling out this form does not make you a member. Dues must be paid and volunteer hours requirements must be met for both 1st & 2nd Semester.
