此报名表仅用于2015年3月13日布鲁克大学中国学生学者联盟会（BUCSSA）的体育之夜报名。

The Sports Night includes Men’s Basketball 3V3, In-door Soccer 5V5, Table Tennis, Badminton (Men’s Singles, Women’s Singles, Men’s Doubles, Women’s Doubles), Billiards, and Three Point Shot.
本次体育之夜的项目分别有男生篮球3V3，室内足球5V5，乒乓球，羽毛球（男单，男双，女单，女双），台球，三分球投篮。

The fee for registration: 1. Men’s Basketball: $30/team (membership $20/team); 3-5 players for each team, maximum 8 teams. 2. In-door Soccer: $40/team (membership $30/team); 5-8 players for each team, maximum 2 teams. 3. Table Tennis: SINGLE player $5/person (membership $3/person), DOUBLE players $8/team (membership $5/team). 4. Badminton: SINGLE player, $10/person (membership $5/person) maximum 6 males and 4 females, DOUBLE player $15/team (membership $10/team) maximum 8 teams. 5. Billiards: $15/person (membership $10/person), maximum 20 people. 6. Three Point Shot $2/person.
每项体育比赛的报名费：1. 男生篮球 $30/队 (会员$20/队)；每队3-5人，限报8队。 2. 室内足球$40/队 (会员$30/队)；每队5-8人，限报2队。3. 乒乓球单打$5/人，会员$3/人；双打$8/ 组，会员$5/。组4. 羽毛球 男(女) 单$10/人（会员$5/人）；男单限6人，女单限4人，男（女）双$15/人（会员$10/人），限8组双打。5. 台球：$15/人（会员$10/人），限报20人。 6. 三分球投篮无报名人数上限，$2/人。

**BUCSSA Members have about $5/person $10/team discount with valid membership card ($2/person $3/team for Table Tennis), each membership card only be used once. Any discount is NOT apply for Three Point Shot. Double Badminton players are treated as a team with $5 dollars/team discount.
**凡持有布鲁克中国学生学者联谊会（BUCSSA）有效会员卡的同学将享有$5/人或$10/组（乒乓球$2/人或$3/组）的一次性折扣（如果本人一次性参加多个项目除篮球和足球，也可以享有有效会员卡的优惠），任何折扣不适用于3分球投篮。羽毛球双打的组合将会享有$5的优惠。

Due to the limit space or time for Badminton, BUCSSA would select one or two kind of the badminton from Men’s Doubles, Women’s Doubles, or Mixed Doubles according to the number of team registered for the event. The registration team only needs to pay the registration fee until the final decision make. For other registration fees, please submit to BUCSSA’s Promotion Tables before the deadline.
由于场地和时间的限制，学联会将根据报名情况从羽毛球男双、女双、混双中选取1或2个项目来比赛。所以报名以上项目的同学请等待通知，于比赛前一天项目决定后缴纳相关费用。其他项目的参赛者请在报名截止日期前到学联会的Promotion Table 处缴纳费用。

This registration will be close on 11:59PM Wednesday, March 12th. 2015. Any filled event will be removed from the choices.
报名截止时间为2015年3月12日晚上11点59分，报满的项目将从报名选项中被移除。
