Equipment Use Agreement
ECUMENICAL CAMPUS MINISTRY

A donation is requested for the use of equipment. A $50 refundable deposit will be required at the time of reservation. Damage costs will be charged against the deposit. If damage costs exceed the deposit, renter will be liable for the balance. There will be a 2.2% processing fee applied to all credit card or PayPal/Square/Cash app payments.
PLEASE MAKE CHECKS PAYABLE TO: ECUMENICAL CAMPUS MINISTRY. SEPARATE CHECKS REQUESTED FOR DONATION AND DEPOSIT.

(No dates will be added to the calendar as reserved without the signed agreement and security deposit.)
Donation and deposit are due before equipment is used. Your deposit will be returned to you after equipment is returned, pending forfeiture for damage. Donation schedule:

Tables & Chairs can be returned anytime during building hours, which are 8am-11pm daily. If returning speakers & microphones, a drop off time must be requested for business days (Monday-Friday) between 9am-5pm. For after-hours or weekend drop off of speakers & microphones, please contact ECM before submitting request.
