The purpose of this form is to inform us with details about yourself, not only so we can learn more about you, but to be able to make reservations in advance for the safari portion of the trip. Please note that participation slots are by first-come first-serve basis  **Each person who plans to come must fill out this form for a proper head count (esp. for the safari portion)** is no longer accepting responses.
