Use this form to certify your faith community's lay delegate(s) and alternate(s) to the 2026 ECMN Convention. One person from each faith community should complete this form on behalf of your entire faith community. Do not include clergy; clergy are already certified as delegates to the Convention.

Your faith community's approved number of lay delegates is based on the membership numbers you submitted on your 2023 Parochial Report. Your faith community is eligible to certify as many alternates as delegates. Please refer to the following table to find the number of lay delegates for which your faith community is eligible:

1-99 members = 1 lay delegate
100-199 members = 2 lay delegates
200-349 members = 3 lay delegates
350-499 members = 4 lay delegates
500-699 members = 5 lay delegates
700-899 members = 6 lay delegates
900-1199 members = 7 lay delegates
1200+ members = 8 lay delegates
