Participant Liability Waiver:                                                                           I have submitted work that was selected for inclusion in a current community art exhibit at Lakeshore AND/OR the Spring 2023 Community Art Exhibit, to be hosted by College of Lake County. I agree that the work selected on loan shall remain on display for the duration of the project from March to June.  College of Lake County has the right to photograph/ video or obtain photographs/videos of the selected works for use in exhibition publications or publicity, telecast, website, post on the internet, social media, or for use in educational materials.  I grant permission to College of Lake County on behalf of their agents or employees, to use photographs/video taken of me for use in college publications such as recruiting brochures, catalogs and/or schedules, newsletters and/or magazines, and to use the photographs/ videos on promotional display boards, and to use such images in electronic versions of the same publications or on the Voices of Lake County postcard project website and on college websites and/or other electronic forms or media, and to offer them for use or distribution in other non-college publications such as news media releases, electronic or otherwise, without notifying me.  I hereby waive any right to inspect or approve finished photographs/videos, printed material, or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of photographs/video.  I hereby agree to release, defend, and hold harmless College of Lake County District (and all participating community partners) and their agents or employees, including any firm publishing and/or distributing the finished product in whole or in part, whether on paper, via electronic media, or on web sites, from and against any claims, damages or liability arising from or related to the use of the photographs, including but not limited to any misuse, distortion, blurring, alteration, optical illusion or use in composite form, either intentionally or otherwise, that may occur or be produced in taking, processing, reduction/enlargement or production of the finished product, its publication or distribution. I am (or the signing parent/guardian is) 18 years of age or older and I am competent to contract my own name.  I have read this release before signing below, and I fully understand its contents, meaning and impact of this release.
