Liability Waiver Form 

I understand that there are risks and dangers inherent in participating in outdoor activities and activities involving use of hand tools. I also understand that in order to be allowed to participate, I must give up my rights to blame Boise River Enhancement Network for any damage which I may suffer while participating in outdoor activities, including workshops and workdays. 

Knowing this, and in consideration of being permitted to participate, I hereby voluntarily release Boise River Enhancement Network from any liability resulting from or arising out of my participation in Boise River Enhancement Network sponsored activities. I understand and agree that I am releasing not only the entities set forth in the two paragraphs above, but also the officers, agents, and employees of those entities. 

I understand and agree that this release will have the effect of releasing, discharging, waiving, and forever relinquishing any and all actions or causes of action that I may have or have had, whether past, present, or future, whether known or unknown, and whether anticipated or unanticipated by me, arising out of my participation. This release constitutes a complete release, discharge, and waiver of any and all actions or causes of action against the Boise River Enhancement Network, and its officers, agents, or employees. 

I understand and agree that this release applies to personal injury, property damage, or wrongful death which I may suffer, even if caused by the acts or omissions of others. 

I understand and agree that by signing this release, I am assuming full responsibility for any and all risk of death or personal injury or property damage suffered by me while participating in Boise River Enhancement Network activities, including while in transport to and from any Boise River Enhancement Network activities. 

I understand that and agree that this release will be binding on me, my spouse, my heirs, my personal representatives, my assigns, my children and guardian ad litem for said children. 

I understand and agree that by signing this release, I am to release, indemnify and hold Boise River Enhancement Network, and its officers, agents, or employees harmless from any and all liability or costs, including attorney fees, associated with or arising from my participation in Boise River Enhancement Network coordinated activities.

Consent to be Filmed/Photographed and Use of Image/Voice

I hereby grant to Boise River Enhancement Network, their successors, and their assignees the right to record the image and/or voice and use the artwork and/ or written work of myself and/or my child, listed below, on videotape, on film, on photographs, in digital media and in any other form of electronic or print medium and to edit such recording at their discretion. I understand that myself and/or my child’s full name, address and biographical information will not be made public. I further grant Boise River Enhancement Network, their successors, and their assignees the right to use, and to allow others to use, myself and/or my child’s image and/or voice on the internet, in brochures, and in any other medium and hereby consent to such use. I hereby release Boise River Enhancement Network, their successors, and their assignees and any using myself and/or my child’s image and/or voice, artwork, and/or written work pursuant to this media release form any and all claims, damages, liabilities, costs and expenses which I and/or my child now have or may hereafter have by reason of any use thereof. I understand that the provisions of this release are legally binding.
