Er mwyn cymryd rhan yng ngweithgareddau Gaeaf Llawn Lles bydd angen i chi ddarparu data personol. Llywodraeth Cymru fydd rheolwr y data personol a byddwn yn eu prosesu yn unol â'n tasg gyhoeddus a'r awdurdod a freiniwyd i ni i helpu Gweinidogion Cymru i wneud penderfyniadau sy'n ymwneud â datblygu polisi a gweithgareddau eraill a gynlluniwyd i gefnogi lles plant a phobl ifanc.
Bydd unrhyw ddata personol a drosglwyddir i Lywodraeth Cymru yn cael eu dileu dair blynedd ar ôl y dyddiad y daeth i law.
Byddwn yn defnyddio'ch data personol i wneud y canlynol:
mesur pa mor dda y mae Llywodraeth Cymru a'n partneriaid yn darparu eu gwasanaethau i blant a phobl ifanc;
helpu i wella'r gwasanaethau hyn;
cefnogi ymchwil ehangach i'r gwasanaethau a ddarperir i blant a phobl ifanc (0 i 25 oed);
cysylltu data â ffynonellau data eraill er mwyn gwerthuso effaith y prosiect ar yr unigolion sy'n cymryd rhan.
Byddwn hefyd yn defnyddio'ch manylion i gysylltu â chi i wneud gwaith dilynol a dysgu mwy am eich profiadau, i'n helpu i ddeall effaith y rhaglen. Mae cymryd rhan mewn ymchwil yn wirfoddol ac ni fyddwch yn cael eich adnabod mewn unrhyw adroddiadau.
Bydd y gwerthusiad annibynnol yn cael ei gynnal gan drydydd parti achrededig (e.e. sefydliad ymchwil neu gwmni ymgynghori). Dim ond o dan gontract y bydd unrhyw waith o'r fath yn cael ei wneud. Mae telerau ac amodau safonol Llywodraeth Cymru ar gyfer contractau o'r fath yn nodi gofynion llym ar gyfer prosesu a chadw data personol yn ddiogel.
Os cysylltir â chi i gymryd rhan mewn gweithgareddau ymchwil sy'n gysylltiedig â'r cynllun hwn, byddwch yn cael Hysbysiad Preifatrwydd ar wahân yn amlinellu sut y caiff gwybodaeth a gynhyrchir gan yr ymchwil ei chasglu, ei chadw a'i defnyddio.
Bydd yr holl ddata a gesglir yn cael ei adrodd mewn fformat dienw pan gaiff ei ddefnyddio mewn adroddiadau ystadegol neu ymchwil. Ni fydd unrhyw adroddiadau yn cynnwys eich manylion cyswllt nac unrhyw wybodaeth y gellid ei defnyddio i adnabod unigolion.
Mae Gofal Iechyd Digidol Cymru hefyd yn gallu gweld rhan o'ch data personol am gyfnod o dri mis, nes bod y gwaith o gysylltu data gyda SAIL (Cyswllt Diogel Gwybodaeth Ddienw) wedi'i gwblhau.  Byddant yn defnyddio eich data personol i greu dynodwyr dienw er mwyn gallu cysylltu'n ddienw â setiau data eraill. I wrthwynebu neu gyfyngu ar y prosesu (mewn rhai amgylchiadau);
