This form is only for club leaders to fill out and gives clubs eligibility to vote on IMD meeting topics.  Once you have filled out this form please send a check for the correct amount (based on team size) to:

Club registration fees based on club size:
15 or less athletes - $25
16-30 athletes - $50
31-70 athletes - $75
More than 70 athletes - $100
