Assumption of Risk and Release of Liability and Indemnity Agreement:
Due to the potential and inherent risks involved in the ceremonies, retreats, and sweat purification lodges, all participants, facilitators, and attendees are required to complete, sign, and deliver this Release from Liability and Assumption of the Risk form to Taino Woods Sanctuary before engaging in any activity or entering onto the Property. During their stay at Taino Woods Sanctuary, Jesse Rivera, Chantel Rivera, and a few People will be holding Native American Ceremonies (prayer retreats, prayer events, purification lodges) collectively called “Ceremony” at the property known as Taino Woods Sanctuary SBL: 1-1-84.6 Big Woods Road, Harris NY. 12742 (“Property”).
In addition to the terms and conditions set forth, it is acknowledged that the owners of the property, and their families, as well as any business entity (collectively called Taino Woods Sanctuary or Taino Woods LLC Owner) including but not limited to any directors, officers, employees, agents, and representatives, have absolutely no involvement, participation, responsibility, liability, or duty whatsoever of any kind to any facilitator, supporter, visitor, or attendee. In addition, neither Owners nor renters of the Property have absolutely any involvement, responsibility, liability, or duty whatsoever of any kind to any facilitator, supporter, visitor, or attendee. Any person 18 years of age or under must be named on one or more of the Assumption of Risk and Release from Liability form.
It is requested that parents and/or guardians discuss the risks of participating in any of the activities. The following information is provided to assist in the proper completion of this form. Attendees without the completed and signed form will not be allowed on the Property.

1. Please read the form in its entirety before signing and initialing the form.
2. Parents or guardians must sign the form for each participant under the age of 18 who are attending.
3. Print legibly the name of the signer and the name of each attendee under the age of 18.
4. Date the form on the blank provided below.
I, the undersigned, comprehend, am willfully attending, and prepared for the activities I am undertaking, such as ceremonies and sweat purification lodges; and I fully understand that such activities involve many inherent and external risks. I know and accept that by participating in such activities, I risk personal injury or even death from many causes including but not limited to: slips, trips, falls, or other injuries; bug, insect, or other animal bites; heatstroke, sunburn, dehydration, dizziness, nausea and/or medical conditions, known or unknown. I know that risks exist on the Property and that the Property has been left in its natural condition, therefore, no one has made any representations nor assurances that the Property is safe or fit for any use, purpose, or reason. I understand that the ceremonies and activities will be under the direction and supervision of Chantel Rivera or agent Jesse Rivera, Taino Woods LLC officers, and that neither the Property Owners, Facilitators, nor renters have any duty, responsibility, or liability whatsoever to any person. I acknowledge that my attendance and participation in the ceremonies on the Property is not at the invitation or request of either the Owners or the Tenants, but it is my own free choice and will. I understand that I could get hurt or injured and that if I do not wish to assume the risk of injury, I do not have to attend the ceremonies and activities.
Therefore, I freely accept and fully assume any and all risk that I may get hurt or injured while on the property, not only in the ways described above but also in ways that are unknown and/or unexpected and/or even unanticipated; and even if I follow the instructions or advice of Chantel Rivera or Taino Woods Sanctuary’s officers, I acknowledge that Chantel Rivera has no affiliation nor relationship of any kind with the facilitators and/or Guests. I hereby waive and release any and all claims of any kind, known and unknown, that I or my heirs have or may have in the future against the Facilitators and/or Guests for any loss, damage, expense, or injury, including death, suffered from or in connection with my presence on the Property, or participation in the ceremonies due to any cause whatsoever including but not limited to negligence on the part of the Facilitators or Guests.
I hereby relieve Chantel Rivera Owner and Taino Woods Sanctuary Inc. Tenants from any duty to protect me from harm, and agree that even if Chantel Rivera Owner, or Taino Woods Sanctuary Inc. Tenant, chooses to implement, suggest, or require safety procedures, such actions shall not alter the fact that Chantel Rivera Owner nor Taino Woods Sanctuary Inc. Tenant have no duty whatsoever to protect me or make the Property safer or more secure.
I will hold harmless and indemnify Chantel Rivera Owner and Taino Woods Sanctuary Inc. Tenant for any liability for property damage or personal injury, including death, to myself and any other person resulting from or arising in connection with any activity on the Property.
I have read and understood the foregoing, and have discussed it with my children, parents and/or guardian; and I am voluntarily signing the Agreement below. If I am signing on behalf of any minor child, I represent and warrant that I am doing so with the consent and approval of the other parent of my child and I understand that I am acknowledging the risks to my child. I have read and understood this Agreement and intend that it be binding on me, my heirs, executors, administrators and assigns. By signing this Agreement, I intend to waive legal rights against Chantel Rivera the Owners and renters on behalf of myself, my heirs, executors, administrators, and assigns.
