The Office of Catechesis and the Ministry with Persons with Disabilities wish to assist parishes in providing sacramental prep and faith formation for children (and adults) with disabilities.  

The information collected from this survey will be used to:
 - connect individuals across the diocese working with individuals with disabilities
 - create a database of lesson plans, helpful resources, and individuals for support
 - create "best practices" for serving children (and adults) with disabilities
 - provide guidance on the formation offered by the Office of Catechesis
