A disability and diversity access needs form for online (Zoom) CPD events learning (master classes)  

Dear Participants,

We are committed to providing an inclusive and accessible learning environment for all participants during our online Zoom CPD events. Your feedback and insights are invaluable to us as we strive to make reasonable adjustments to ensure a positive and enriching experience for everyone.

This form is designed to gather information about any specific accessibility needs or accommodations you may require to fully participate in the webinar. To assist us in personalizing your learning experience, we would appreciate your voluntary provision of your name and email.
