Please complete this form if you are a BIO major that is looking to enroll in AMS 110 with AMS 102/AP credit on your academic record. This is the only time this form can be utilized for enrollment.

The following requirements have to be met prior to completing this form:

Your enrollment appointment (date and time) has already occurred.
You can not have any course conflicts with the AMS 110 course that you are requesting
You have not met the current credit limit and the addition of this course will not exceed the current credit limit

If the requirements are not met at the time of submission, your form will not be processed.

Requests are generally reviewed within 4-5 business days (but can take longer, especially during peak periods) If approved, students will be enrolled in the AMS 110 course. If denied, student's will be notified of denial.
