WFYI Survey: Have you or a loved one been affected by sudden cardiac arrest? Would you know what to do if you were

WFYI, Indianapolis’ public radio station, is working on a series of stories about sudden cardiac arrest and we would like to hear from you. We want to talk to people across the state who have been personally affected by sudden cardiac arrest to tell their stories. We also want to understand what questions the community has about cardiac arrest and what may be done to help. Please respond to the survey by Dec. 4. 

How this information will be used: We will not use your identifying information in a story without your expressed permission. This survey is anonymous unless you choose to share your name and you would like to participate in a future listening session or interviews.

More about the reporter: I started looking into this topic after the death of a 15-year-old my friend volunteered with. I've since written a feature story on the difficulties of mandating automated external defibrillators (AEDs) in schools. Now I want to spread more awareness about sudden cardiac arrest, in youth and adults, and how to respond to it.
