As the parent or guardian of the registered child(ren), we are aware that all athletic activities involve the potential for injury. We acknowledge that even with the best instruction, use of good equipment and strict observance of rules, injuries are still a possibility. We will not hold any of the coaches or staff liable for any injuries that may occur as a result of camp or clinic related activity. We also give permission for the Certified Trainer on staff to apply first aid and/or arrange for emergency transportation if needed. As a parent or guardian, we carry proper medical insurance for our child(ren). Also, we give permission for our child to be photographed and for the photographs to be used in media and publications.
