2013-2014 U of L Rotaract Club Membership Form
Welcome to Rotaract!

Rotaract can be described in 3 words: Help, Learn, Enjoy.

Rotaractors in 8,400 clubs across 170 countries make small and large differences in people's lives around the world every day. We do it through projects and activities with humanitarian outcomes; as we plan and deliver these projects, our members gain valuable experience and form invaluable relationships, in both personal and professional capacities. Rotaractors also know how to have a good time.

Don't be overwhelmed by the amount of stuff our club does; think of Rotaract as a large buffet of opportunities to choose from; you are not expected to participate in everything, but we hope you participate in all of the activities that interest you. To keep up to date on these activities, make sure you're on the club email list, and check us out online.

***Purpose of this Form***

Whether you are a new member or a returning member, we want to hear from YOU.

This year, we are focusing on serving and supporting our club members as much as possible by doing the projects you want to do and supporting the organizations you want to support. This form is one way you can help steer the club by telling us who you are, what you want to do, and what your passions, skills, and needs are. Club and event planning meetings are other ways you can help steer the club, so keep an eye out for those opportunities as we announce them.

***Membership Criteria***

Rotaract is an open, fun, and welcoming place for anyone interested in making a positive impact on the world; everyone is always welcome to join our meetings and events, regardless of the amount of time they can commit.

The purpose of membership criteria is to ensure the benefits of membership are provided to regular participants, such as: voting / running in club elections, resume references, eligibility for scholarships specific to this Rotaract club, financial subsidies for Rotary/Rotaract events/travel, etc. Below are the (very achievable) criteria to qualify as a member in good standing:

- Submitting the Membership Form (below)
- Participation:
     (1) 4 club functions / semester, which can include:
          - Volunteer, fundraiser, training, or social events
          - Club, committee, board, or executive meetings
          - Anything that our club hosts
     (2) One Rotary function / year, which can include:
          - Rotary club meetings
          - Volunteer, fundraiser, or social events
          - Training, conferences, festivals
          - Anything that Rotary hosts

***Use of Information***

Information collected in this form will be used strictly for the operation of the University of Lethbridge Rotaract Club; it will be shared with the club's Board of Directors, Executive Committee, and other club leaders as needed to better enable them to make your experience as a member of the Rotaract club as awesome as possible. We value and respect your privacy the same as ours (we fill out the form too).

All questions are optional (except for your name and email address because we use the responses for our membership list) but the more you tell us, the better we can serve you. This survey is intended to be confidential but not anonymous, so that we can connect you and your interests with specific opportunities throughout the year.

***Photographs***

As a member of the club, your photo may be taken at any club function, and may be used in club promotional material.
