2026 Fort Bragg Quilt Show Entry Form
NOTE: PLEASE read the 2026  FBQS INFO FOR ENTRIES before completing this form (see oceanwavequilters.com). Deadline for completing registration and submitting payment is JUNE 1, 2026.

All quilts are due at Sew ‘n Sew on Monday, June 16, 2026 between 10am and 4pm.
  
You will need to fill out one form for each of your quilts. When you have finished entering your quilt, HIT "SUBMIT" at the bottom of the last section.

RELEASE OF LIABILITY: I expressly agree to release Ocean Wave Quilters from all liability, loss, damage, theft, injury and destruction to my property during setup and display at the 2026 Fort Bragg Quilt Show. I understand and agree that insurance for my entry(ies) is my own responsibility. I consent to have my quilt or entered items photographed. *
Quilts are not considered "entered" until payment is received. $15 for up to 3 entries; $5 for each additional entry (you may pay for ALL your quilts at once). 
(NOTE: Fees are waived for Featured Artist and Special Exhibit quilts.) Make checks payable to Ocean Wave Quilters.
