In accordance with Article 6(1)(a) of the GDPR, I hereby give my consent to the Pavol Jozef Šafárik University in Košice, Šrobárová 2, 041 80 Košice, ID No.: 00397768 (the Controller) to process my personal data to the extent specified in this binding application form for the purpose of organizing and implementing the FutureTeach 2025 scientific conference. This consent is valid for the duration of the process of preparation and realization of the conference at the Department of Pedagogy of the Faculty of Arts of the University of Pavol Jozef Šafárik in Košice. I am aware that the provision of personal data as well as the consent to their processing is voluntary. I can withdraw my consent at any time by sending a written withdrawal of consent to the address of the controller. Withdrawal of consent is effective from the date of receipt. I have been informed of the rights which Chapter III Rights of the data subject regulates the obligations of the controller in exercising the rights of data subjects set out in Articles 12 to 22 of the GDPR.
