One of the best ways to measure our effectiveness in the community is to solicit citizen comments regarding police department services. We encourage you to use this form to register your suggestions, commendations, or complaints.

If you are completing this form to file a commendation involving a department employee, a copy of this form will be placed in the employee's personnel file.

Each complaint will be thoroughly investigated. Then warranted by facts, appropriate corrective action will be taken.
