Tylenol Permission 24.25 SY

'Per WES Physician Standing Orders: Tylenol may be given after written consent by parent/guardian, as needed, for pain, menstrual cramps, etc.' The parent/guardian will be notified via phone or email when Tylenol is administered. Tylenol may only be given three times per 30 Calendar days. 

Tylenol will not be given for fevers or other acute illnesses without the written order of the student's primary care provider.

I give permission for the school nurse to administer Acetaminophen (generic Tylenol) to my child. 

*I understand that this will be given for headaches, menstrual cramps, or minor muscle aches/soreness for the school year 2024/2025. To my knowledge, my child has no allergy/sensitivity to this medication and I am not aware of any reason why it cannot be taken at this time.
*I will let the nurse know if my child has taken any over the counter medications prior to arrival at school.
