Enterprise Architecture Survey
Introduction
You are invited to take part in a research study being conducted by William Donaldson and Dr. Thomas Mazzuchi of the Department of Engineering Management and Systems Engineering, George Washington University (GWU).

You are being asked if you want to take part in this survey study because of your subject matter expertise in systems engineering, specifically Enterprise Architecture Frameworks. Please read this form and ask us any questions that will help you decide if you want to be in the study. Taking part is completely voluntary and even if you decide you want to, you can quit at any time.

Purpose
This research looks at the efficacy and scalability of Enterprise Architecture Frameworks (EAF).

Procedures
The total amount of time you will spend in this study is 15 – 20 minutes.

Risks & Confidentiality
The study has the following risks:

There is a small chance that someone not on our research team could find out that you took part in the study or somehow associate a name with the information we collect about your enterprise, however the following steps are being taken to reduce this risk.
• No identifiable information such as name, address, employer, race or sex is being collected
• Each survey respondent will only be assigned a number for coding purposes
• The responses to the survey are stored on a password protected website only accessible by research team
• The results will be downloaded for analysis onto a password protected laptop which is stored in secure office of a researcher
• Once the survey is completed the data will be deleted from the secure website

The records of this study will be kept private. In any published articles or presentations, we will not include any information that will make it possible to identify you as a subject.

Your records for this study may be reviewed by departments of the University responsibility for overseeing research safety and compliance.

Benefits
Taking part in this research will not help you directly; however the benefit to society will be a better understanding of systems engineering competencies.
