MIND Mentor Application 2025 - 2026
If you are interested in becoming a MIND Mentor for the 2025 - 2026 school year, please complete the questions below by September 1st. Mentors should review the MIND Mentor Information (visit https://parkcityreads.org/mind/) and be in grades 9-12. Previous participation in MIND programs is required for students in grades 9. 

Past Mentors do not need to reapply.

Acceptance into the Mentor Program is dependent on space available.

Our first MIND Mentor meeting, which includes our required annual orientation, will be on Wednesday, September 3rd from 5:30 - 7:00 pm in the PC READS Office. Will you be able to attend? *
This year, we are hosting an after school program at Ecker Hill once a month on Tuesdays from 3:45 - 5:00 pm. Mentors will be expected to participate in a minimum of 4 meetings and will register for these separately, but please indicate your expected availability. *
