RSO Registration

This form is to be submitted annually to the Office of Student Life in order to establish or retain active status, or when any changes in the information occur during the academic year. The form should be submitted upon resuming normal club activities in the fall semester, prior to using any club funds, or whenever any of the information below changes. The form must be completed by the organization's president (or equivalent), or one of its advisors.
