Thank you for your participation in this year's programs. In an effort to continually improve our organization, please complete the following Leader Evaluation. Check all responses that you feel are accurate. Each section will have a comments portion, to clarify your choice. Keep in mind all aspects of the leader's role, including preparations prior to, during, and after your program. Your evaluation is confidential and will not be shared with the individual, but will help the chapter in an overall assessment, to help us continue to improve upon our leader criteria each year.
