Dear VITAL THOUGHT Participants:

Welcome! All classes are real time and interactive online via Zoom or Google Meet. You will receive invite emails with all necessary links from from your instructor(s) at least 3 days before your course(s) begin.

Your seat in any purchased course is held for you and confirmed once we receive and process this application. If you have not yet purchased a course, please do so BEFORE submitting this form, unless you are requesting free enrollment.

Please submit this form ASAP to confirm your enrollment in one or more VITAL THOUGHT insight courses. You must submit a form each time you purchase enrollment. If you enroll in more than one course at once than only one form is needed. Later enrollment will require another application submission. No one is denied enrollment based on responses to this application! It is for informational purposes only.

Purpose: 
Our application allows us to learn more about how to best serve our participants during this and future sessions. The information gathered here is shared with VITAL THOUGHT administrators for purposes of future course curation, marketing, organizational development. Some limited information is shared with instructors--like names, pronouns, location-- so that course instructors can personalize and optimize their teaching. We do not share this information with any parties outside of VITAL THOUGHT.

Enrollment Fee Waiver: Would you like to request a fee waiver? If so please provide more information about your interest and need. Also please list the courses you want to take in order of priority. For every 3 paying Participants 1 free seat is opened. Up to 3 free seats per course. Free seats are granted on a first come basis, starting with those who have submitted this Application. If not filled we then open it to our email list, then on social media.
Course Selection: Select all classes you have enrolled in (i.e. purchased through our website.) This will not affect your payment or enrollment, it is only for our record keeping. If you are applying for a Course Fee Waiver, select in which courses you would like to enroll if/ when a free seat opens.

Education Level/ Background: What is your educational background? We welcome people from all educational backgrounds. This information is used to help instructors personalize the courses most effectively and will help us reach future participants. Check the answer that best describes your education and all that apply. Select a degree if you are currently enrolled in it.
Education Area: If you are currently in school or have completed one or more degrees or other educational or training programs, please indicate your area(s) of study or training (e.g. your major, minor, degree program, research area, professional area, trade, etc.).
Occupation: What is your current or most recent occupation, profession, or industry? We welcome the employed, unemployed, underemployed, students, retired, etc. Feel free to list multiple jobs, vocations, and/or careers. This information is used to help instructors personalize the courses most effectively and will help us develop future course offerings and marketing strategies.
Future Course Schedules: In future VITAL THOUGHT sessions would other scheduling structures work better for you? If so, what would be ideal? Everyone's schedules are different so we want a sense of how to best serve our participants. Select all that apply, and enter alternatives in "Other."
