Food Approval For Student Event

Any food purchased for students using university funds requires approval from the college controller. Unlike the event approval form that also asks about food, this food approval is financial in nature and is to insure that university funds are being used for a legitimate purpose. Although both approvals ask about food, the approvals are unrelated and serve different purposes, so both are required.  

This form is to be filled out at least one week before your event or activity.

Purchasing Food For Your Event
1. Once this information is submitted, a request to the college controller will be made. Then, you will be informed if it has been approved. 
2. Go to the front desk and check out a credit card.
(Note: If you are buying something on campus (Food to Go, BYU Store, BYU Catering, etc.) ask for a campus card. If you are purchasing off campus, request a credit card.)
3. Fill out the credit card usage contract.
4. Leave your driver’s license or student ID card as collateral.
5. Take a blank expense form and the card.
6. We have one credit card and multiple users, so please return the card immediately after completing your purchase. Please note that the card cannot be kept overnight.
7. After you have made your purchase, fill out the expense form, attach an itemized receipt to the form, and return it with the card. Please note that your ID card will not be returned to you until you bring back a receipt.
