Please realize that dogs and cats need vet care throughout their lives, including yearly boosters, heartworm, flea/tick preventative meds, etc. If you cannot afford a donation, can you afford the appropriate vet care?
I certify the above information is accurate and complete to the best of my knowledge. I authorize the release of veterinary information related to my past and current pets. I give AARF permission to contact the persons listed above for character reference. Any incomplete or inaccurate information will be the grounds for denial of your application.
I agree to treat the animal humanely and will provide proper food, water, and will keep the animal as an indoor pet. I agree to provide veterinary care as needed. I agree to obtain the proper municipal license required under local law. I will not sell this animal or allow this animal to be used for experimentation in any way. I will not allow this animal to run at large. If I find for any reason that i can not keep this animal I will return the animal to Ashland Animal Recue Fund. I understand the ability of AARF to accept the animal back at any time will be based on capacity and other criteria, and may take several days to arrange.

We do not guarantee or assume responsibility for temperament or behavior of animals adopted from us, nor can we be responsible for any infectious diseases that could be incubating in any animal at the time of adoption.
