I agree that if my child exhibits unsafe behaviour he or she may be disqualified to receive childcare. *
I agree that childcare is only offered to children whose parents are in the gymnasium or in their Loon Lake rooms (i.e. not hiking or canoeing in the lake). *
I agree that my failure to cooperate with childcare providers may disqualify my child's attendance without refund. *

I hereby authorize the Zawiyah Foundation to photograph and/or otherwise record images and/or sounds of or including my child that may be used for advertising or social media.

BY STATING YES BELOW, I THE PARENT/GUARDIAN, AM THE PARENT AND/OR LEGAL GUARDIAN OF THE CHILD AND FOR THE CHILD I HEREBY CONSENT TO HIM OR HER PARTICIPATING IN THE ACTIVITY, AND, FOR MYSELF AND THE CHILD, IN RETURN FOR THE ZAWIYAH FOUNDATION ALLOWING THE CHILD TO PARTICIPATE IN THE ACTIVITY:

I NOW WAIVE ALL CLAIMS AND HEREBY RELEASE the Zawiyah Foundation from any and all liability for any complaints, demands, claims, actions, suits, judgements and orders for any and all losses, injuries, damage or expenses incurred or experience in connection with the Child's participation.
