Application form for Confirmations
Please fill out the details of the person who is to be Confirmed - from now on referred to as the "candidate".

In addition to filling out this form, the Latin Mass Society requires copies of the following items:

1. A copy of the Baptismal Certificate of the candidate (and reception into the Roman Catholic Church, if applicable).
2. A letter/email from a Roman Catholic Priest confirming that the candidate is suitably instructed to receive the Sacrament of Confirmation.

These items must be submitted either by email - info@lms.org.uk, or by post to 11-13 Macklin Street, London WC2B 5NH. The deadline for the submission of these documents is Friday 23 October 2015.

Failure to provide these will result in rejection of the application.

A donation to the Latin Mass Society to defray the costs would be greatly appreciated (£50 is suggested). You can make a donation on the LMS website, using the donate button. Or send a cheque to the above address.

It is vital we have the full Baptismal details of the candidate. We will send them a certificate of your Confirmation in order for them to update their records.
