Vendor Approval Request Form
Thank you for your interest in adding a vendor to the GVCS approved vendor list. Please make sure that you read and understand the Vendor Approval Guidelines in the next section prior to requesting a new vendor. These guidelines may also be found at www.goldenvcs.org/vendors. All requirements must be met prior to submitting the vendor for approval.

This Vendor Approval Request Form asks for a link to the prospective vendor's website. If the vendor does not have a website, please submit a professional brochure, catalog, or flyer by mail to the school office or by email to GVCSvendors@goldenvcs.org. This/these document(s) must present (1) the name of the business, (2) the services or materials provided, and (3) the approximate cost of the services or materials. The submitted items will not be returned.
