KCCC Youth Ministry Visitor Form
The following form is for any adult (college student, young adult, and/or parent aged 18+) auditing or observing an official KCCC Youth Ministry event, activity, or ministry. All adults must agree to every agreement or will be asked not to participate.  This form is to be filled out only with approval by the Director of Youth Ministry of Katy Christian Community Church at their own discretion and shall be submitted each and every time auditing or observing a KCCC Youth Ministry event or activity.

Please note:
  1. Your involvement and participation may be limited in scope, specifically in access and contact with Youth, in accordance with our internal youth ministry protection policies.
  2. We ask that you do not take any photos and/or videos of Youth.  Posting pictures or video of Youth on any platform is prohibited.
  3. We ask that Visitors refrain from engaging in one-on-one contact with Youth outside the supervision of Youth Workers.  This includes in-person contact and communication as well as connecting on social media, direct messaging on any platform, text, etc., both during and after the ministry event.
  4. Do not expose Youth to any inappropriate media, social media posts, materials, images, etc.  If it’s questionable, err on the safe side.
  5. We ask that you do not bring any weapons, firearms, drugs, or drug paraphernalia into KCCC Youth Ministry events and activities.
  6. Unless given explicit permission by the Youth Ministry Director, no food, snacks, or drinks are to be distributed or given to Youth at any KCCC Youth Ministry event or activity.
  7. Foul or unwholesome language will not be tolerated.  All communication, both verbal and digital, should be appropriate in content and magnitude.

Responsibility of Conduct:

I understand that I am responsible for my conduct and behavior during the program or event and will conduct myself in a Christ-like way that glorifies God, protects the Youth under the care of KCCC Youth Ministry, and is above reproach, including my dress, speech, physical activity, and any and all interactions with minors must these actions must be above reproach. I further agree that if I engage in any unlawful actions (including but not limited to illegal substance use, vandalism, theft, or any other criminal behavior), inappropriate activity, or any act that hinders the purpose and/or function of KCCC Youth Ministry during the event, I may be subject to immediate removal from the event premises and/or be reported to the appropriate authorities, at the discretion of the Director of Youth Ministry. In such cases, I understand that I forfeit any rights to refunds or compensation, and I may be held liable for any damages or costs incurred by KCCC and it's members as a result of my actions.

Understanding of Risks:  I understand that observing youth group activities may incur certain risks and dangers. I am aware that these activities may include, but are not limited to, group games, sports, outings, and other recreational activities that may have inherent risk.

Voluntary Participation:  My participation as a Visitor is entirely voluntary, and I acknowledge that I am not required to be present during these activities.

Assumption of Risks: I understand that there are inherent risks associated with the activities and hereby assume full responsibility for any injury, damage, or loss that I may sustain as a result of my presence and participation as an observer.

Clear selection
Release and Waiver of Liability: In consideration for being allowed to observe youth group activities at Katy Christian Community Church, I hereby release and discharge Katy Christian Community Church, its employees, agents, volunteers, and representatives from any and all claims, liabilities, actions, causes of action, and demands that may arise as a result of my presence and participation in the youth group activities. This release and waiver apply to all forms of injury, damage, or loss, whether physical or otherwise.
