Stony Brook University Hospital - Southampton has been able to obtain the COVID vaccine for another Shelter Island vaccination pod.  The dates of distribution are Wednesday, April 7th for the first dose and Wednesday April 28th for the second dose.  If you cannot commit to being available on both dates please do not sign up to be on this list. Shelter Island School and the Town of Shelter Island rely on the google suite platform for submission and collection of data and the submitter's personal internet provider for the connection to this platform and are not responsible for submission errors due to either.

THIS IS THE WAITLIST, if you are selected, you will be called with a time.  There is a consent form that needs to be filled out prior to receiving the vaccine . The form is available at https://coronavirus.health.ny.gov/system/files/documents/2021/02/vaccine-consent-form-2.12.21-0917.pdf  The survey also includes the link to the consent form which should be completed, printed and brought to the vaccine appointment if selected.
