I freely consent to attend Rootedness, Renewal and Reciprocity led by Kelly Dignan (also known as AWARE CO, LLC) and Allison O'Neall. I am responsible for my own growth and experience, and I determine the effectiveness of the workshop content. This workshop is not psychotherapy or counseling. It does not address mental disorders as defined by the American Psychological Association. This workshop is not a substitute for counseling, psychotherapy, mental health care, substance abuse treatment or medical care. If I am struggling from a medical or psychological perspective, I will access professionals in these fields. I am 100% responsible for myself, the outcomes of this workshop, my choices and behaviors. Information provided during the workshop and the follow on individual session is confidential unless there is reasonable cause to know or suspect self-harm, that a child or elderly person has been subjected to abuse or neglect, or that a child or elderly person is being subjected to circumstances or conditions that would reasonably result in abuse or neglect. The relationship with the facilitators is a professional relationship in which sexual intimacy is never appropriate. Payment in full ($120) reserves my spot in the workshop. Before August 28, 2021, cancellation in writing and a 50% refund is available. After August 28, no refund is available. I acknowledge that I understand the above information.
