CONSENT TO PARTICIPATE

Introduction
• You are being asked to take part in a research study. We ask that you read this form and ask any questions that you may have before agreeing to be in the study.
Purpose of Study  
• The purpose of the study is to investigate the Factors Affecting Productivity and Retention and its Impact on Organizational Performance in an Insurance Sales Call Centre.  The research will be published as part of an MBA Dissertation.  
Description of the Study Procedures
• If you agree to be in this study, you will be asked to truthfully answer all the questions in this survey.  It should take you no more than 15 minutes to complete.  Management has given permission for the time required to complete this questionnaire.
Confidentiality
• This study is anonymous.  We will not be collecting or retaining any information about your identity, and  we cannot include any information in any report we may publish that would make it possible to identify you as a participant.
• The records of this study will be kept strictly confidential and is not accessible to anyone other than the researcher. Research records are submitted and kept electronically, on a secure server, and  information will be coded and further secured using a password protected file.
Payments
• There will be no payment for participating in this study.
Right to Refuse or Withdraw
• The decision to participate in this study is entirely up to you.  You are also entitled to withdraw your response from the study, after having submitted it.  The survey requests that you provide an identifier email, which is only requested in order to identify your response if you want to withdraw it from the study.  You may refuse to take part in the study at any time without affecting your relationship with the company.  Your decision will not result in any loss or benefits to which you are otherwise entitled.
Risks/Discomforts of Being in this Study
• There are no reasonably foreseeable risks or discomfort associated with taking part in this research.
Right to Ask Questions and Report Concerns
