I, the undersigned, understand that participation in Takshanuk Watershed Council's summer camps involves certain risks, including but not limited to physical injury, stings/bites, or property damage. I voluntarily assume all risks associated with my child’s participation in camp activities.

I hereby release, waive, and discharge Takshanuk Watershed Council's employees, and volunteers from any and all liability, claims, or causes of action arising out of any loss, damage, or injury that may be sustained by my child while participating in camp activities, whether caused by the negligence of the releasees or otherwise. 

In the event of an emergency, I hereby grant permission to Takshanuk Watershed Council staff to secure proper medical treatment for my child. I understand that I am responsible for all costs related to medical treatment and transport.

I certify that my child is physically fit and has no medical conditions that would prevent full participation in the camp program, except as noted in the medical section of this form or discussed with TWC staff prior to registration.
